Job Summary


The Division of Pediatric Nephrology at the University of Utah School of Medicine is seeking a Clinical Nurse to function as a member of the nephrology team. The nephrology team consists of 2 RN and 2 APC's working with 6 MD's. This position will be responsible for delivering professional nursing care to patients via phone, health portal, Tele Health and Tele Home, and in outpatient settings. Utilizes strong organizational and leadership skills to assess patient status; implements clinical nursing interventions to modify symptoms; develops, implements, coordinates, and evaluates patient care plans and patient education materials to enhance and improve outcomes and ensure a safe and therapeutic environment.

Responsibilities

Essential Functions
  1. Performs phone management with patients' parents, chart review, care coordination, triage, and problem management. Document pertinent information in patients chart.
  2. Discusses patient and family concerns over the phone and/or via the electronic health record.
  3. Provides and develops patient/family education materials surrounding common nephrology disorders.
  4. Communicates adjustments in treatment plans, including medication adjustments, to patients and families.
  5. Evaluates patient response to interventions and modifies plan of care as needed to achieve health care goals in conjunction with patient's providers.
  6. Develops and participates in educating patients and their families about care programs.
  7. Communicates lab & other test results to families.
  8. Assist with medication refills.
  9. Assists providers and staff with patient-related paperwork such as letters of medical necessity, DMV forms, school care plans, and pre-authorizations.
  10. Maintains a safe and therapeutic patient care environment.
  11. Works closely with central triage team to provide high quality, efficient care to patients and their families.
  12. Keeps current on advances in nursing care by participating in educational programs.
  13. Orients and educates new staff in division and departmental procedures.
  14. Utilizes knowledge of medications, procedures, infection control, and safety precaution, and initiates appropriate interventions in crisis situations.
  15. Participates in total quality management to positively affect patient and system outcomes to improve care.
  16. Works with other parts of Clinical Enterprise (Triage, Scheduling, Referrals) to optimize patient care.
Incumbent will be expected to provide high quality patient care in an organization focused on patient safety, patient and parent satisfaction, and positive patient outcomes; advocates for patients and families to ensure unique needs are met; collaborates with highly skilled team members dedicated to achieving patient goals and providing world class care; actively participates in educational opportunities. Education in patient management is ongoing with the help of division members.

This position involves intensive work that may exert up to 100 pounds and may consistently require lifting, carrying, pushing, pulling or otherwise moving objects, such as medical equipment, or patients while providing medical care. Workers in this position may be exposed to infectious diseases and may be required to function around prisoners and behavioral health patients.

The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate knowledge of the principles of life span growth and development and the ability to assess data regarding the patient's status and provide care as described in the department's policies and procedures manual.

Minimum Qualifications

Required
Current RQI Healthcare Provider eCredential through the University of Utah Health RQI system. The eCredential is to be obtained within 30 days of hire.
Current license to practice as a Registered Nurse in the State of Utah, or obtain one within 90 days of hire under the interstate compact if switching residency to State of Utah. Must maintain current Interstate Compact (multi-state) license if residency is not being changed to Utah.
Preferred
Some positions may require additional certifications such as Advanced Cardiac Life Support ( ACLS), Pediatric Advanced Life Support ( PALS), or Neonatal Resuscitation Program ( NRP) within one year of hire.
This position is patient-sensitive and must fulfill all associated requirements.
