PALLIATIVE CARE INTAKE/SCHEDULER COORDINATOR

A full-time, Monday-Friday 8a-5p, Palliative Care Intake/Schedule Coordinator position is available. The Palliative Care Coordinator will manage new referrals and scheduling. Primary responsibility will be to the Palliative Care department, however cross coverage within the organization is expected. The Palliative Care Intake Coordinator/Scheduler will process all new referrals, schedule patient visits, and communicate with healthcare providers. This position will also be responsible for maintaining up-to-date records and communications within the electronic medical record. 

This position requires excellent interpersonal and communication skills. Attention to detail and computer skills are necessary, along with strong time management skills.
