Hospice Aide (CNA) - FT Days 7a-7p - Nashville Inpatient Unit

Schedule - 7 a.m. - 7 p.m., rotating schedule



SUMMARYThe Hospice Aide is a member of the IDT who provides personal care and addresses personal needs under the direction, instruction and supervision of a nurse. Works to promote the patients comfort and assist in providing a safe patient environment. 

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Provides personal care including bathing, hair, skin and nail care, shaving, feeding, dressing, toileting, changing bed linens, etc.
  2. Treats each patient with dignity, respect, and observes their rights to privacy and confidentiality.
  3. Partners with the nurse to meet the needs of the assigned patients and families.
  4. Follows the instructions / directions of the Clinical Director and Charge Nurse.
  5. Provides emotional support to patients and families.
  6. Assists in maintaining a clean and safe environment in the patient rooms.
  7. Informs nurse of changes in patient condition or patient needs.
  8. Works to prevent skin breakdown
  9. Completes documentation per procedure including personal inventories on admission.
  10. Follows the hospice care plan as developed by the IDT.
  11. Remains in assigned work area unless directed otherwise by the nurse.
  12. Gives report to oncoming Hospice Aide including changes in patient condition.
  13. Assists with distribution of dietary trays to patients and returns trays to kitchen when patient is finished.
  14. Follows Alive Hospice policies and procedures and maintains current standards of practice.
  15. Involves volunteers in patient care as appropriate and works to enhance the volunteer's experience.
  16. Follows Infection Control procedures when providing personal care.
  17. Passes the Hospice Aide Competency test on an annual basis.
  18. Orient new Hospice Aide as requested.
  19. Other duties may also be assigned.

QUALIFICATIONS

CONTINUING EDUCATION

The agency requires the position to complete 12 hours of continuing education per year covering topics that will contribute to improvements in carrying out the above responsibilities. Regulatory agencies may require some disciplines to have additional hours in order to be licensed or certified. 


EDUCATION and/or EXPERIENCE

High school diploma or GED. Minimum of one year direct patient care or related experience. Professional maturity and ability to respond calmly and effectively in stressful situations. 

  

LANGUAGE SKILLS 

Ability to read and interpret treatment orders and plans, labels on containers, Safety Data Sheets etc. Ability to write documentation of care delivered and result of interventions. Able to speak effectively to peers, patients, families and supervisors relaying important and accurate information.


MATHEMATICAL SKILLS 

Ability to apply mathematical concepts such as ratio, percent, measurement and comparison. Ability to weigh, measure and convert to metric or centigrade.


REASONING ABILITY 

Ability to use common sense and good judgment to prioritize and problem solve within context of direct care orders from professional staff. Able to demonstrate emotional maturity, calm and compassionate demeanor. Able to identify risk and plan for safe delivery of services. Able to distinguish basic normal physiology from abnormal. Able to follow geographic directions, as well as, complex direct care and record keeping directions.


OTHER QUALIFICATIONS

Requires Nurse’s Aide Skills, observation and communication skills and knowledge of hospice care. Requires understanding and ability to clean patient areas according to standard process and good infection prevention practices. Good physical and mental health. Caring, compassionate, tact, patience and good personal hygiene. Reliable transportation is also a requirement. Able to use EMR (Electronic Medical Records).

CERTIFICATES, LICENSES, REGISTRATIONS 

Current Tennessee CNT Certificate 

Current and valid Tennessee driver’s license and automobile insurance as required by Tennessee State Law.
